Network180 Outpatient Clinic


The Network180 Outpatient Clinic is housed within the Network180 Integrated Behavioral Health Center, providing a comprehensive array of mental health and substance use disorder services to vulnerable adults, youth, and families. The multidisciplinary team includes therapists, clinical specialist, psychiatrist, registered nurse, medical assistant and a clinical supervisor/program manager.  After hours on-call support is offered through the Network180 Access Line. 

The Outpatient Clinic utilizes evidence-based treatment interventions to help educate participants about their behavioral health needs and engage individuals to play an active role in their care. Therapists complete comprehensive biopsychosocial assessments as well as person centered plans with the people they work with. Special attention is given to social determinants of health, utilizing a culturally responsive and trauma-informed approach.

All Outpatient Clinic employees are trained in Motivational Interviewing, and therapists are trained in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT). Services are tailored to the youth and caregiver’s stage of change and utilize family-driven, youth-guided principles.

The Clinical Specialist will further support needs that impact the participant's overall health and wellness, including those related to social determinants of health. These needs may range from additional psycho-education and crisis response to connection to formal and informal supports, including but not limited to: financial benefits, food, clothing and household goods, short-term shelter, long-term housing and housing subsidies, medical, dental and vision benefits/care, transportation, increasing social support networks, and support and liaison services for youth and/or caregivers hospitalized or in juvenile detention/jail or other restrictive settings. Additionally, the Clinical Specialist will coordinate services and treatment planning with other systems – probation, education, child welfare and primary care physicians.

The type and frequency of services varies from person to person and time to time based on the needs of the individual. Services are both office-based and community based and determined by the individual's needs and choices.

As part of the Integrated Behavioral Health Center, the Network180 Outpatient Clinic maintains a high level of care coordination with primary care as well as increased access to services at a variety of different levels.  The Outpatient Clinic team meets weekly for huddles to provide an opportunity for key communication and collaboration as well as a proactive approach to treatment.

Additionally, at least one therapist each will be trained in the assessment and treatment of co-occurring substance use disorders and the treatment of co-occurring intellectual/developmental disorders.

Participation in inpatient and crisis services will be tracked, and discharge plans are created to provide a follow-up appointment with a therapist within 7 days of discharge. Psychiatric follow up will occur within 30 days. These plans will be strengthened to make contact within 24 hours for those at risk of suicide or for those being discharged from crisis settings. 

The type and frequency of services varies from person to person based on the needs of the person served. Services are provided in an office or community setting and at a time that is most convenient for person-served, including day and evening hours. Outpatient clinical services for established clients seeking an appointment for routine needs will be provided within 10 business days of the requested date for service. Those presenting with an urgent need will be scheduled within 1 business day of the request.

Eligibility

The Network180 Outpatient Clinic serves individuals of any age experiencing mild to moderate mental illness and/or co-occurring disorders. 

The Network180 Outpatient Clinic allows self-referrals and coordinated transitions from Network180’s existing provider network. Additionally, individuals will be identified by our 24-hour Access Center or Crisis Services and via our team of co-located, clinical liaisons within hospitals, child welfare, criminal justice systems.

  • Monday-Thursday 8:00 a.m. - 7:00 p.m.
  • Friday 8:00 a.m. - 5:00 p.m. via telehealth

Early morning and weekend appointments are available as needed. Same day appointments are available daily for all providers. Our Crisis Services are available 24/7 for those who may be experiencing a psychiatric crisis by calling (616) 336-3909 or dial/text 988.


Scope of Care Coordination

The Network180 Outpatient Clnic will provide care coordination with primary care and offer assistance in obtaining a primary care physician if the individual does not currently have one. Coordination with primary care is done primarily by the Clinical Specialist and includes: ongoing and timely exchange of information (e.g. medication lists), advocacy, transportation assistance, chronic disease education, and coordination with health plans. Referrals to primary care or other specialists are done in accordance with the choices of the caregiver as well as the recommendations of the team's providers.

The entire team working in the Network180 Outpatient Clinic are trained in Motivational Interviewing and cross trained in “physical” chronic health conditions such as diabetes and hypertension. Blood pressure and BMI are taken at every visit with the psychiatrist, and labs are ordered routinely based on health status and medications prescribed.

Discharge/Transition Criteria

Potential reasons for discharge include but are not limited to:

  • The person reaches the goals they set and the individual and/or caregiver no longer wants to remain in services.
  • The person decides to receive behavioral health care elsewhere.
  • Individual has not received services in the clinic for 90+ days and/or have a repeated pattern of missed appointments
